We are searching for a Clinical Services Partner to join our Transitional Care team based in Bedford, Nova Scotia. Shannex aims to build capacity in our health system by improving access and flow and effective resource utilization. The Goal of Transitional Care is to provide care that focuses on the patient's functional ability and support network, providing the necessary support and resources to transition back to community, either returning to their prior living disposition or another care setting. In partnership with Nova Scotia Health (NSH), we are excited to open and operate the first Transitional Care Centre in Nova Scotia. Reporting to the Director, Clinical Services, the Clinical Services Partner, Transitional Care works collaboratively with the Transitional Care Team, Shannex's Clinical Services Team, and members of the NSH Team, to provide quality patient care consistent with applicable legislation, Accreditation standards, evidence-based practices, and policies and procedures. Researches and provides evidence-based recommendations for nurses and members of the Transitional Care Team to support safe and effective transitions in care for adults (18 years and older) and their support network. Works closely with Shannex and NSH to identify clinical education needs for employees and supports with developing a clinical education plan for roll out. Delivers clinical and IPAC training sessions, workshops, and educational programs and monitors effectiveness. Participates in development, evaluation and revision of clinical policies, procedures, and education consistent with legislation, best practices, and Accreditation standards.
