*Job Title:* Case Manager
*Supervised by:* Program Supervisor, Case Management
*Qualifications: *
BA or Certificate/Diploma in a related field and a member of a regulated health profession required. Over 5 years of experience in mental health services. Good understanding and ability to use concepts, principles and specific techniques of the psychiatric rehabilitation model. Advanced knowledge and ability to apply a high level of clinical counselling skills. Understanding of psychiatric diagnostic categories and related symptoms. Thorough knowledge of mental health system and the Landlord and Tenant Act. Basic understanding of the Mental Health Act, housing for special needs populations, use of computers and community development. Able to exercise moderate judgment where procedures and protocol have not been set. Able to form strong relationships with clients who have a serious mental illness, often complicated by addictions, non-compliance, homelessness and/or forensic issues. The relationship needs to be at a therapeutic, behaviour change level, often done on an outreach basis and characterized by closeness, trust, confidentiality and a non-judgmental attitude. Able to work cooperatively with internal and external colleagues to develop a team approach. Good communication skills of a clinical standard. Valid drivers license and reliable car. Eligible to work in Canada. Fully vaccinated for COVID-19 (2 doses).
*Hours of Work: *
This department operates from 9 am to 5 pm 6 days a week and holidays. The normal work week is 40 hours.
Flexibility in these hours is required and expected to allow for vacation and sick leave coverage, staff training, public relations, client/agency emergencies, special agency functions and other responsibilities.
*Agency Responsibilities: *
* Ensures at all times the confidentiality of client and agency information.
* Conducts all matters in a professional manner with respect to the individual dignity and right to self-determination of all clients.
* Establishes relationships within and outside of the agency in a professional manner so as to enhance the reputation of the organization and facilitate the rehabilitation and community acceptance of persons with mental illness.
* Conducts all matters in a punctual and responsible manner.
* Works co-operatively with other staff for the good of the agency and the rehabilitation of our clients.
* Exhibits trustworthy behaviour in matters related to agency/client finances and property.
* Maintains accurate and up-to-date records, reports and statistics as required by the agency.
* Participates in staff meetings, agency committees and external committees as appropriate.
*Specific Responsibilities:*
*Direct Service:*
* Work with 12-15 high need clients which may include case coordination, liaison with other professionals and family members, life skills support, monitoring, advocacy, and/or developing a rehabilitation plan that consists of, but is not limited to, an overall rehabilitation goal, individual functional assessment, direct skill teaching/programming, plus discharge plan and follow-up.
* Primary focus of interventions is stable housing and behaviour change to increase independence
* Resolving immediate presenting problems (no family physician, lost medication, eviction notice, finding local resources, help completing forms, etc).
* Supportive counselling, and instrumental assistance and crisis intervention for clients of the Support for Independent Living Program, provided primarily in the client?s home.
* Provide back-up support and relief coverage to team members including the brief service worker.
* Provide group recreational opportunities for the program clients.
* Facilitating application to specialized housing such as OHC, HSC, non-profit housing, group homes, etc.
* Work collaboratively with various partners such as Shelter Now, The Guest House, other Housing Providers.
