For more information, visit www.oxmed.ca. *Qualifications* * Current Nurse Practitioner license in Ontario * Master's degree in Nursing or equivalent * Experience as a Nurse Practitioner in a clinical setting preferred * Experience in case management, patient care, medication administration, home care, hospital settings, dementia care, or medical office is preferred * Strong clinical assessment skills and ability to make independent decisions regarding patient care * Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members * Ability to work collaboratively in a team environment * Proficiency in using electronic health records (EHR) *Duties* * Provide comprehensive primary care to patients, including physical examinations, diagnosing and treating common acute illnesses and injuries, and managing chronic conditions * Develop and implement patient care plans in collaboration with the healthcare team * Perform diagnostic tests and interpret results to aid in diagnosis and treatment decisions * Prescribe medications and monitor patient response to medication therapy * Educate patients and their families about health conditions, treatment options, and preventive care measures * Coordinate care with other healthcare providers and specialists as needed * Maintain accurate and up-to-date medical records Note: This job description is not intended to be all-inclusive.
