Our pharmacy is currently seeking a licensed and registered pharmacist with the Ontario College of Pharmacists to join our team, focusing on providing patient coaching and management for medication checks, diabetes management, asthma, COPD, and other health programs. Responsibilities: Practice within provincial guidelines and uphold the Code of Ethics and Standards of Practice. Provide comprehensive counseling on all prescriptions and health topics to ensure positive patient outcomes. Perform expanded scopes of practice, including minor ailments, medication reviews, and pharmaceutical opinions. Conduct detailed medication checks and patient coaching sessions, particularly focusing on chronic disease management (diabetes, asthma, COPD). Collaborate effectively with allied healthcare professionals to ensure holistic patient care. Maintain accurate records in accordance with legislative requirements.
