*Integrative Nurse Practitioner, Cornerstone Health Centre* *Georgetown, Ontario* *Job Overview* Cornerstone Health Centre in Georgetown is seeking a Nurse Practitioner to join our professional staff. This is a part-time contract position with flexible options for virtual or in-person appointments. You would be fully supported by excellent administrative staff who do private billing and patient management. We are specifically seeking an individual with an interest or experience in Integrative and/or Functional Medicine, or a willingness to learn this approach to health care through mentoring. *Requirements*: * Nurse Practitioner licensed in Ontario * Minimum of 2 years of clinical experience, ideally with a focus on functional or integrative medicine * Strong communication skills, empathy, and commitment to patient-centered care * A keen interest in lifestyle medicine, finding the root cause and prevention. * Willingness to step away from provincially funded health care towards private billing *Responsibilities* * Conduct thorough patient assessments and evaluations to determine health status and needs. * Develop and implement individualized treatment plans based on patient assessments. * Provide primary care services, including diagnosis and management of acute and chronic illnesses. * Collaborate with multidisciplinary practitioners such as Naturopaths, Psychotherapists, Massage Therapists and Chiropractors. * Educate patients and their families about health conditions, treatment options, and preventive measures. * Maintain accurate medical records using electronic health record systems * Monitor patient progress and adjust treatment plans as necessary * Participate in case management activities to facilitate appropriate referrals and follow-up care.
