*Nurse Practitioner Job Description* *Overview* Maple Family Health Team has an exciting opportunity for newly graduated or experienced Nurse Practitioners who are interested in working in primary care in supportive, well-resourced medical clinics. Two positions are available: * 2024-NP-02: this is a temporary (approximately 12 months) position working in a clinic with four physicians. The schedule is flexible, ranging from 2-4.5 days per week depending on your availability. * 2024-NP-03: this is a permanent part-time position working alongside one physician in the care of their patient roster of approximately 1,300. This is an excellent opportunity to learn or become re-acquainted with primary care general practice under the supportive guidance of a physician. *About Maple Family Health Team* Please visit our website for more information about our clinics and programs. *Duties* * Conduct thorough patient assessments and develop individualized care plans. * Provide direct patient care, including medication administration and case management. * Monitor patients' progress and adjust treatment plans as necessary. * Collaborate with physicians, nurses, and other healthcare staff to ensure optimal patient outcomes. * Educate patients and their families about health conditions, treatment options, and preventive care. * Order laboratory tests and interpret results to guide clinical decisions. * Participate in quality improvement initiatives to enhance patient care services. *Experience* * Current registration with CNO as a Nurse Practitioner * Evidence of liability insurance through CNPS or other reputable agency * Demonstrated experience in community and/or primary care medicine * Leadership, organization, research, evaluation, time management, communication, critical thinking and interpersonal skills * Ability to work independently and within a team environment * Proficiency in the use of computers and electronic medical record systems. Additional information about the positions * The incumbent is eligible for HOOPP * vacation time starts at 4 weeks' per year * The incumbent must be compliant with Maple's COVID vaccine policy which requires full vaccination, * Compensation is commensurate with experience and is compliant with the amount set by the Ministry of Health for Nurse Practitioners in Family Health Teams.
