Qualifications : Bachelor's Degree in social workCurrently registered with NLCSW2 years current experience (within the last 5 years) in an addiction, and/or mental health settingAdvanced clinical knowledge and skills in the areas of assessment, intervention, case planning and community engagementSpecific knowledge and experience in the areas of mental illness, addictions, and its impacts on individuals and familiesDemonstrated ability in interviewing and crisis intervention techniquesDemonstrated leadership ability in the areas of interdisciplinary collaboration, advocacy, mentorship and program planningDemonstrated analytical, as well as oral and written communication skillsDemonstrated independence, initiative, and ability to work with othersWilling to work a flexible work schedule, may include evenings and weekendsUse of a private vehicle and valid driver license are a condition of employmentSatisfactory Record of work performance and attendance is required for this positionmust be eligible for registration with the Newfoundland and Labrador Association of Social Workers.The successful applicant will be required to submit a satisfactory certificate of conduct Duties :The FACT Coordinator functions in a leadership role within the interdisciplinary Flexible Assertive Community Treatment (FACT) team with duties including: referral managementliaising with referral agentschairing daily FACT meetingscoordinating the daily schedulecoordination of Psychiatry clinics and/or telehealth sessionsconsultation and collaboration with internal and external service providers and familycoordination and monitoring of treatment plans and service deliverywill be responsible to provide clinical leadership to other team memberswill assist with program planning and development, evaluation and change managementWhile the primary function of this position is to support operations of the FACT Team, the Social Worker II will be involved in assessment, treatment, follow up and support of individuals with mental health and addictions needs throughout various mental health and addictions programs and services.Other related duties as required; travel may be required; may be required to work alone.- LocationHappy Valley-Goose Bay, NL
