<p>The selected candidate will provide primary care services to a panel of clients and families (0-99), in a transition structure to the new care model (MIC-M).</p><p>SUMMARY OF THE POSITION<br>Person who does design, guidance, counselling, implementation, analysis and assessment work in one or more social programs.</p><p>SPECIFIC FUNCTIONS<br>The Community Miyupimaatisiiun (health) Centre (CMC), offers front-line services and community health for the local population of Chisasibi. Services are given where it is best suited in accordance with the clients' needs and are offered through various means such as consultation at the CMC, home visit, group activities and support with community resources.* Works as part of an interdisciplinary team and more specifically in close partnership with Community Health Representative (CHR), Community worker, Nurse, Social Assistance Technician, Doctor, Nutritionist, Midwives, and various other community health services and staff;* Carries out psychosocial assessment, care, counselling, information and referral services for women and family in order to resolve psychosocial and emotional problems and allow the clienteles to increase their self-reliance, quality of life and social integration; * Supports different community working relationships including participation, liaison and consultation;* Liaise and refer to other resources within the CMC and other community entities and resources.* Conduct psychosocial assessments, interventions and follow-ups with clients;* Organize group interventions as well as outreach activities to support community members; * Carries out all other related tasks upon the request of her/his immediate supervisor;* Performs any other related tasks.</p><p>REQUIREMENTS<br>Education: * Bachelor's Degree in Social Work or Social Services;* Member in good standing of the Ordre professionnel des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec (OTSTCFQ).<br>Experience:</p><ul><li><p>Two (2) years of relevant experience in social work.<br>Knowledge and Abilities:</p></li><li><p>Good understanding of psychosocial issues;</p></li><li>Knowledge of Cree culture, social/health issues and Eenou/Eeyou Pimaatsiiun healing practices and paradigms is an asset;</li><li>Ability to identify and analyze clientele needs; </li><li>Ability to communicate theoretical and practical knowledge, as applied to coaching and training;</li><li>Excellent critical thinking, synthesis, organizational and decision-making skills; </li><li>Sense of accountability, resourcefulness and autonomy; </li><li>Teamwork skills.</li><li><p>Excellent autonomy and adaptation skills; <br>Language:</p></li><li><p>Fluent in English and French;</p></li><li>Fluency in Cree is an asset.<br>Other: </li><li>Includes on-call period.</li></ul>- LocationMontréal, QC
